Abstract:

Introduction and Objectives:
Escherichia coli and Klebsiella pneumoniae are one of agent’s nosocomial infections. Bacterial spread in hospital during hands contact with contamination surfaces due to and therefore accrue nosocomial infections. β–lactame antibiotics are important to cure of nosocomial infections. Spread b-lactamase in Bacteria with inactive this antibiotics and due to disorder therapy of patients.

Materials and Methods:
The research was laboratory and performed during of 2005/2007 years in Azzahra hospital in Isfahan, According to statistical formula randomly selected 374 sample. Environmental samples collected, with swab in TSB and staff hand samples collected with Finger Print method and clinical samples randomly selected. Bacterial identification, were performed with microbiological methods. For determine β–lactamase production, use Acidimetric method, and Antibiogram pattern were performed with Kirby Bauer method.

Results:
According to Acidimetric test results respectively 100%, 85.7%, 68.6% of  Escherichia coli strain and 100%, 75% and 73.3% of Klebsiella pneumoniae strain isolated from hands ,surfaces and clinical samples, production β–lactamase . According to Antibiogram test respectively 42.4%, 32%, 26.8%, 30%, 33.3%, 8.7%, of Escherichia coli strains and 76.9%, 28.6%, 25%,35%,25, and 20% of  Klebsiella pneumoniae  strains were resistant into Penicillin G, Ampicillin,  Cephotaxime, Ceftriaxoen, Cefipime and Imipenem.

Conclusion:
High prevalence β–lactamase in Bacteria represent inadequate and un necessary use of b-lactame antibiotics. According to sensitivity Bacteria into third and fourth generation cephalosporin’s and carbapenems, we offer for cure patients, this antibiotics replacement other β–lactame antibiotics.
